The consent letter itself
It should identify the child, both parents, the travelling adult, the destination and dates, and state the consent explicitly. The signature of the non-travelling parent is certified by the notarial attorney, with a copy of that parent's passport attached.
Where custody is not shared
Provide a certified copy of the court order or the registered agreement establishing sole custody, translated where the destination requires it. A statement by the travelling parent alone is not treated as proof.
Guardians and relatives travelling with a child
Both parents normally sign, and some destinations also want proof of the relationship and of accommodation arrangements at the destination. Confirm the airline's own policy separately from the immigration rule.
